Sonographic Significance of Infantile Hypertrophic Pyloric Stenosis

Abstract
This study is a clinical analysis of the 58 consecutive cases of infantile hypertrophic pyloric stenosis (IHPS), that was surgically treated at the Department of Pediatric Surgery, Kwangju Christian Hospital, from Jan. 1989 to Dec. 1992.
@ES The results are as follows:
@EN 1) The most prevalent age group was between the 3 weeks and 5 weeks of age, and male to female ratio was 10.6:1.
2) In all cases, nonbilous projectile vomiting was noted.
3) The average level of each electrolytes were as follows.
4) Na+" 136.1 mEq/L, K+:3.37 mEq/L, Cl+:97.6 mEq/L operation. Average symptomatic period was 6.5 weeks when serum K+ level was below 3.2 mEq/L (4 cases-6.9%), and 3 weeks when serum Cl level was bclow 88 mmEq/L (8 case-13.8%).
5) When serum K+level was below 3.2 mEq/L (4 cases-6.9%), the average thickness of pyloric muscle was 5.7mm and the length was 19mm, and compared with all cases studied, the pyloric muscle thickness was thicker but the length was similar. When
serum
Cl-level was below 88 mEq/L (8 cases), the average thickness of pyloric muscle was 4.7 mm and the length was 19mm, and compared with all cases studied. no difference was noted.
6) In the preoperative ultrasonography, the average thrckeness of pylorus was 5.1 mm and the length was 18.4mm
7) Ultrasonographically normal level of pyloric muscle thickness was noted to be 3.5 mm and the lenght 15mm. And 26 (87%) in 23 cases showed recovery to normal level 1 month after operation, and remained 3 cases recovery to normal level 2 months
later
after operation.
